ABSTRACT

Objectives: 1. define the occurrence of work-related hearing problems and voice disorders among teachers that have contacted the Colombian National Board of Disability Assessment (NBDA) for follow-up; 2. identify individual associated factors of hearing problems and voice disorders among teachers; 3. assess the limitations and restrictions due to hearing problems and voice disorders among these participants. Methods: Retrospective study. The National Database of the Colombian NBDA was reviewed. Information on distributions of occupation, individual characteristics, and diagnosis code (ICD-10) was analyzed. Results: Communication disorders among teachers that have contacted the Colom-bian NBDA for follow-up included voice disorders, with a prevalence of 51%, and hearing problems, with a prevalence of 7%. Female teachers who have contacted the Colombian NBDA for follow-up were 4 times more likely to be identified as having voice disorders compared with their male colleagues. Conclusions: While teachers that have contacted the Colombian NBDA for fol-low-up have a high occurrence of voice disorders, hearing problems are more likely to be stated as a debilitating condition. One possible explanation is that teachers who contacted the Colombian NBDA for follow-up continued working even when many voice symptoms were evident, while hearing problems would prevent a teacher from interacting with students, thereby affecting the teaching-learning process soon-er. Nevertheless, with both voice and hearing problems, work performance and social interaction is affected, and, therefore, quality of life is reduced.


Objetivos: 1. definir la ocurrencia de problemas de audición y de voz relacionados con el trabajo de docentes que contactaron la Junta Nacional de Evaluación de la Discapacidad de Colombia (NBDA) para su seguimiento; 2. identificar los factores individuales asociados a los problemas de audición y voz entre los profesores; 3. eva-luar las limitaciones y restricciones debidas a problemas de audición y voz asociados al trabajo entre los participantes.Métodos: estudio retrospectivo. Se revisó la Base de Datos Nacional de la NBDA colombiana. Se analizó información sobre distribuciones de ocupación, característi-cas individuales y código de diagnóstico (CIE-10).Resultados: Los trastornos de comunicación entre los docentes que han contacta-do a la NBDA colombiana para seguimiento incluyeron problemas de la voz, con una prevalencia del 51%, y problemas de audición, con una prevalencia del 7%. Las pro-fesoras que se han puesto en contacto con la NBDA colombiana para el seguimiento tenían 4 veces más probabilidades de ser identificadas con trastornos de la voz en comparación con sus colegas masculinos.Conclusiones: Si bien los maestros que contactaron a la NBDA colombiana para el seguimiento tienen una alta incidencia de trastornos de la voz, es más probable que los problemas de audición se consideren una condición debilitante. Una posible ex-plicación es que los maestros que se comunicaron con la NBDA colombiana para el seguimiento continuaron trabajando incluso cuando muchos síntomas vocales eran evidentes, mientras que los problemas de audición evitarían que un maestro inte-ractuara con los estudiantes, afectando así el proceso de enseñanza-aprendizaje. Sin embargo, tanto con los problemas de voz como de audición, el desempeño laboral y la interacción social se ven afectados y, por lo tanto, la calidad de vida se reduce.

RESUMO Objetivo Monitorar o sistema auditivo central de crianças nascidas pequenas para a idade gestacional, por meio da avaliação eletrofisiológica da audição, para verificar a ocorrência de eventuais disfunções neurais nesse sistema. Métodos Estudo longitudinal, cuja casuística foi composta por 23 crianças distribuídas em quatro grupos: 1) grupo de quatro crianças nascidas pequenas para a idade gestacional e a termo; 2) grupo de sete crianças nascidas pequenas para a idade gestacional e pré-termo; 3) grupo de quatro crianças nascidas com peso adequado para a idade gestacional e a termo; 4) grupo de oito crianças nascidas adequadas para a idade gestacional e pré-termo, cuja idade ao final da pesquisa foi de 3 anos (variação entre 34 e 39 meses). O critério de inclusão foi presença bilateral de emissões otoacústicas transientes. Todas as crianças foram submetidas ao potencial evocado auditivo de tronco encefálico ao nascimento, aos 6 meses e aos 3 anos de idade e à pesquisa do potencial evocado auditivo de longa latência aos 3 anos. Resultados crianças nascidas pequenas para a idade gestacional e a termo tiveram maior ocorrência de alterações, em relação aos demais grupos, com aumento da latência das ondas III e V e interpicos I-III e I-V. Todas apresentaram resultados normais no potencial evocado auditivo de longa latência. Conclusão Crianças nascidas pequenas para a idade gestacional e a termo apresentam disfunções na condução neural no tronco encefálico e devem ser consideradas de risco para alterações do desenvolvimento das habilidades auditivas necessárias para garantir qualidade de processamento da informação acústica.


ABSTRACT Purpose To follow up the central auditory system of children born small for gestational age, through electrophysiological evaluation of hearing, in order to verify the occurrence of possible neural dysfunctions in this system. Methods A longitudinal study was carried out with 23 children divided into four groups: Term-born group, subdivided into small for gestational age (four children) and four children born with appropriate weight for gestational age, whose age at the end of the research was three years old. Preterm group subdivided into small for gestational age (seven children), and appropriate for gestational age (eight children), whose corrected age, at the end of the research was three years old. All children were subjected to assessment of auditory brainstem auditory evoked potentials at birth, at six months and at three years of age, and Long-Latency Auditory Evoked Potential at three years. Results children born at term and small for gestational age had a higher occurrence of hearing alterations in relation to the other groups, with increased latency of waves III and V and interpeaks I-III and I-V. All children presented normal evaluation in the Long-Latency Auditory Evoked Potential. Conclusion Children born term and small for gestational age present dysfunctions in neural conduction in the brainstem and should be considered at risk for alterations in the development of the auditory skills that are necessary to guarantee quality of acoustic information processing.

ABSTRACT

Visual or hearing disabilities account for 20% of people reporting some sort of disability. We performed a literature review about the interaction that people with visual or hearing disabilities have with the health care system. We found that these people report a lower quality of life and have a higher frequency of physical and psychological ailments. They also have difficulties to obtain an adequate health care and report a paucity of trained professionals to take care of them.

ABSTRACT

Abstract Introduction Several studies show correlations between congenital toxoplasmosis and hearing loss, with a broad diversity of levels of hearing loss and specifications of hearing disorders. Objective To describe the studies found in the literature regarding hearing disorders in congenital toxoplasmosis. Data Synthesis A literature review was conducted on the Lilacs, SciELO, PubMed and Scopus databases by combining the following keywords: congenital toxoplasmosis and hearing. Based on this search strategy, 152 papers were found, the majority published on the Scopus and PubMed databases from1958 to 2015. After theapplication of the inclusion criteria, 8 articles published between 1980 and 2015 were included in the present study. Conclusion This review showed a moderate evidence of the association between hearing disorders and congenital toxoplasmosis, which is characterized by sensorineural hearing loss. However, there are gaps in the description of the specific characteristics of the type and level of hearing loss, or of other possible disorders involved in the auditory processing.

ABSTRACT Objective: To investigate early detection of amikacin-induced ototoxicity in a population treated for multidrug-resistant tuberculosis (MDR-TB), by means of three different tests: pure-tone audiometry (PTA); high-frequency audiometry (HFA); and distortion-product otoacoustic emission (DPOAE) testing. Methods: This was a longitudinal prospective cohort study involving patients aged 18-69 years with a diagnosis of MDR-TB who had to receive amikacin for six months as part of their antituberculosis drug regimen for the first time. Hearing was assessed before treatment initiation and at two and six months after treatment initiation. Sequential statistics were used to analyze the results. Results: We included 61 patients, but the final population consisted of 10 patients (7 men and 3 women) because of sequential analysis. Comparison of the test results obtained at two and six months after treatment initiation with those obtained at baseline revealed that HFA at two months and PTA at six months detected hearing threshold shifts consistent with ototoxicity. However, DPOAE testing did not detect such shifts. Conclusions: The statistical method used in this study makes it possible to conclude that, over the six-month period, amikacin-associated hearing threshold shifts were detected by HFA and PTA, and that DPOAE testing was not efficient in detecting such shifts.


RESUMO Objetivo: Verificar a detecção precoce de ototoxicidade causada pelo uso de amicacina numa população tratada para tuberculose multirresistente (TBMR) por meio da realização de três testes distintos: audiometria tonal liminar (ATL), audiometria de altas frequências (AAF) e pesquisa de emissões otoacústicas por produto de distorção (EOAPD). Métodos: Estudo longitudinal de coorte prospectiva incluindo pacientes de ambos os sexos, com idade entre 18 e 69 anos, com diagnóstico de TBMR pulmonar e que necessitaram utilizar amicacina por seis meses em seu esquema medicamentoso antituberculose pela primeira vez. A avaliação auditiva foi realizada antes do início do tratamento e depois de dois e seis meses do início do tratamento. A análise dos resultados foi realizada por meio de análise estatística sequencial. Resultados: Foram incluídos 61 pacientes, mas a população final foi constituída de 10 pacientes (7 homens e 3 mulheres), em razão da análise sequencial. Ao se comparar os valores das respostas dos testes com aqueles encontrados na avaliação basal, foram verificadas mudanças nos limiares auditivos compatíveis com ototoxicidade após dois meses de tratamento através da AAF e após seis meses de tratamento através da ATL. Entretanto, essas mudanças não foram verificadas através da pesquisa de EOAPD. Conclusões: Ao se considerar o método estatístico utilizado nessa população, é possível concluir que mudanças nos limiares auditivos foram associadas ao uso da amicacina no período de seis meses por meio de AAF e ATL e que a pesquisa de EOAPD não se mostrou eficiente na identificação dessas mudanças.

RESUMO Objetivo Identificar os efeitos da ingestão de álcool na gestação sobre o sistema nervoso auditivo central em relação aos seus possíveis diagnósticos, Síndrome Fetal do Álcool, Síndrome Fetal do Álcool Parcial, Distúrbios ao Nascimento Relacionados ao Álcool e Distúrbio do Neurodesenvolvimento Relacionado ao Álcool, sua extensão e o método de avaliação auditiva. Estratégia de pesquisa Busca sistemática e integrativa nas bases de dados PubMed, LILACS e SciELO, com os termos em português e inglês “síndrome fetal do álcool”, “desordens relacionadas ao uso do álcool” associadas a “audição”. Critérios de seleção Dos 123 resumos identificados, foram seis selecionados, publicados até maio de 2015. Análise dos dados Foram elencados tópicos a serem respondidos, caracterização da casuística; o diagnóstico decorrente da exposição fetal nas crianças; método de avaliação auditiva; e resultados descritos. Resultados Entre as avaliações comportamentais, foram utilizados os testes dicóticos verbais com sílabas e com sentenças e o teste fala com ruído. Entre os testes eletrofisiológicos, no Potencial Evocado Auditivo de Tronco Encefálico, foi detectada alteração de sincronia neural, e no Potencial Evocado Auditivo de Longa Latência – P300, valores de latência precoces. Conclusão Existem evidências de que as crianças e adultos jovens expostos ao álcool na gestação apresentam sinais de comprometimento do sistema nervoso auditivo central, mas não foi possível caracterizar essas alterações nos diferentes subtipos diagnósticos do espectro. As vias auditivas corticais foram as mais investigadas e o método eletrofisiológico o mais utilizado, com um resultado inesperado em dois deles, a latência precoce da N2 e da P300.


ABSTRACT Purpose To identify the effects of alcohol intake during pregnancy on the central auditory nervous system in relation to their possible diagnosis, Fetal Alcohol Syndrome, partial Fetal Alcohol Syndrome, Alcohol-Related Birth Defects and Alcohol-Related Neurodevelopmental Disorder, his extension and the hearing assessment method. Research strategy Systematic and integrative review searched the databases PubMed, LILACS and SciELO, with terms in Portuguese and English “fetal alcohol syndrome”, “alcohol-related disorders” associated with “hearing”. Selection criteria: We identified 123 abstracts, six were selected and published until May 2015. Data analysis Were listed topics to be answered, characterization of the sample; the diagnosis result of fetal exposure; method of hearing assessment and described results. Results Among the behavioral assessments, Verbal Dichotic Tests with syllables and sentences and Speech in Noise Test, were used. Among the electrophysiological tests, the Brainstem Auditory Evoked Potential was detected change neural synchrony, and Long-Latency Auditory Evoked Potential – P300, early latency values. Conclusion There is evidence that children exposed to alcohol in utero present central auditory nervous system involvement signals, but it was not possible to identify the influence of different subtypes and their losses. Cortical auditory pathways were the most investigated and the electrophysiological method as used with an unexpected result in two of them, early N2 and P300 latency.

ABSTRACT

As crianças com fissura labiopalatina geralmente apresentam alterações fonoaudiológicas com manifestações em vários aspectos, especialmente no da comunicação. No que se refere à audição, os bebês que nascem com fissura no palato tendem a apresentar acúmulo de fluido na orelha média, devido ao mau funcionamento do mecanismo de abertura e fechamento da tuba auditiva. O quadro pode evoluir para otites, que é umas das causas mais comuns de perda auditiva em crianças com fissura labiopalatina com até 10 anos. Esta perda auditiva geralmente é do tipo condutiva bilateral. Sabe-se que a audição normal é essencial para a aquisição da linguagem oral e efetiva comunicação verbal, e que déficits do sistema auditivo, congênitos ou adquiridos afetam a transmissão e a percepção do som. Qualquer perda auditiva oferece privação sensorial, podendo, assim, levar a alterações em diferentes habilidades auditivas. São crescentes os estudos científicos relacionados às habilidades auditivas em crianças com fissura labiopalatina, contudo, existe uma escassez de trabalhos relacionando habilidades auditivas centrais com as alterações de fala na fissura labiopalatina. Assim, hipotetizou-se que as habilidades auditivas centrais em crianças com fissura labiopalatina que apresentam alterações de fala seriam diferentes das habilidades das crianças com fissura labiopalatina sem alteração de fala e também que poderia existir uma relação entre as alterações de fala relacionadas à Disfunção Velofaríngea e às habilidades auditivas centrais. Este trabalho teve por objetivo verificar a associação entre as habilidades auditivas centrais e alterações de fala decorrentes da Disfunção Velofaríngea (hipernasalidade e emissão de ar nasal), e Articulações Compensatórias em crianças com fissura labiopalatina operada. Nesta pesquisa, foi realizado um estudo prospectivo de 45 pacientes, subdividos em 3 grupos. Todos matriculados no Hospital de Reabilitação de Anomalias Craniofaciais da Universidade de São Paulo, com fissura labiopalatina operada. Foram averiguados inicialmente em prontuário dados quanto à Disfunção Velofaríngea e ao uso de Articulações Compensatórias a fim de compor os três grupos do estudo, sendo o G1 com alterações de fala decorrentes da Disfunção Velofaríngea e Articulações Compensatórias, o G2 com alterações de fala decorrentes da Disfunção Velofaríngea, porém sem Articulações Compensatórias, e G3 (grupo controle) sem alterações de fala decorrentes da Disfunção Velofaríngea e sem Articulações Compensatórias. Posteriormente, os sujeitos foram submetidos à avaliação audiológica periférica e a testes do processamento auditivo central. A Articulação Compensatória de maior ocorrência foi a golpe de glote, seguida pela fricativa faríngea e plosiva dorso médio palatal. O G1 foi o grupo que apresentou o maior número de sujeitos com habilidades auditivas alteradas, seguido pelo G2, e G3. Foi encontrada significância estatística na associação do grupo com alterações de fala decorrentes da DVF e AC com as habilidades de figura-fundo e ordenação temporal. A habilidade de resolução temporal esteve alterada em toda amostra estudada.(AU)


Children with cleft lip and palate usually have speech-language disorders with manifestations in various aspects of communication and supply. With regard to auditing, children with cleft palate tend to have fluid buildup in the middle ear due to malfunction of the opening and closing mechanism of the Eustachian tube. The table may develop into Otitis, which is one of the most common causes of hearing loss in children up to 10 years with cleft lip and palate. This hearing loss is usually conductive type and bilateral. Normal hearing is essential for the acquisition of oral language and effective verbal communication and that deficits of the auditory system, congenital or acquired, affect the transmission and perception of sound. Any hearing loss offers sensory deprivation and may thus lead alteration in different hearing abilities.Scientific studies related to the auditory abilities in children with cleft lip and palate are increasing, however, there is a paucity of studies linking central auditory skills with speech disorders in the cleft lip and palate. Thus hypothesized dry the central auditory skills in children with cleft lip and palate who have speech disorders would be different from the skills of children with cleft lip and palate speechless change and also that could be a relationship between speech disorders related to velopharyngeal dysfunction and central auditory skills .The objective of this study is investigate the association between central auditory skills and speech disorders resulting from the velopharyngeal dysfunction (hypernasality and nasal air emission) and compensatory articulations in children with cleft palate. In this research it performed a prospective study of 45 patients, subdivided into 3 groups. All enrolled in the Craniofacial Anomalies Rehabilitation Hospital of the University of São Paulo, with operated cleft lip and palate. They were initially investigated in medical records data on the velopharyngeal dysfunction and use of compensatory articulations in order to compose the three study groups: the G1 with speech disorders resulting from the velopharyngeal dysfunction and compensatory articulations, G2 with speech disorders resulting from the velopharyngeal dysfunction But without compensatory articulations and G3 (control group) without speech disorders resulting from the velopharyngeal dysfunction and no compensatory articulations. Later the subjects underwent a peripheral audiological evaluation and auditory processing tests. The compensatory articulation was the most frequent glottal stop, followed by pharyngeal fricative and plosive average back palatal. The G1 was the group that had the highest number of subjects with altered auditory skills, followed by G2 and G3. Found statistically significant association between the group with speech disorders resulting from VPD and CA with the figure-ground skills and temporal skills. The temporal resolution skill was altered in all groups of this study.(AU)

ABSTRACT INTRODUCTION: Abnormalities in auditory function of newborns may occur not only because of preterm birth, but also from the use of medications and from diseases related to prematurity. OBJECTIVE: To analyze the inhibitory effect from stimulation of the olivocochlear efferent system on transient evoked otoacoustic emissions in preterm neonates, comparing these data with those from full-term neonates. METHODS: This was a prospective, cross-sectional, contemporary cohort study with 125 neonates, pooled into two groups: full-term (72 full-term neonates, 36 females and 36 males, born at 37-41 weeks of gestational age); and preterm (53 neonates, 28 males and 25 females, born at ≤36 weeks of gestational age, evaluated at the corrected gestational age of 37-41 weeks). Otoacoustic emissions were recorded using linear and nonlinear click-evoked stimuli, with and without contralateral stimulation. RESULTS: The inhibitory effect of the efferent pathway in otoacoustic emissions was different (p = 0.012) between groups, and a mean reduction of 1.48 dB SPL in full-term births and of 1.02 dB SPL in preterm births was observed for the non-linear click-evoked stimulus. CONCLUSION: The results suggest a reduced inhibitory effect of the olivocochlear efferent system on otoacoustic emissions in preterm neonates.


RESUMO Introdução: Alterações na função auditiva de recém-nascidos prematuros podem ocorrer não só devido ao nascimento antecipado, mas também pelo uso de medicamentos e por doenças relacionadas à prematuridade. Objetivo: Analisar o efeito inibitório da estimulação do sistema eferente olivo coclear sobre a amplitude das emissões otoacústicas evocadas transientes em recém-nascidos prematuros, comparando esses dados aos de recém-nascidos a termo. Método: Estudo prospectivo, de coorte contemporânea com corte transversal, com 125 recémnascidos, distribuídos em dois grupos: a termo (72 recém-nascidos a termo, 36 feminino e 36 masculino, nascidos entre 37-41 semanas de idade gestacional), e pretermo (53 recém-nascidos, 28 masculino e 25 feminino, nascidos com idade gestacional ≤ 36 semanas avaliados entre 37-41 semanas de idade gestacional corrigida). As emissões otoacústicas foram registradas a partir de estímulos clique lineares e não lineares, com e sem estimulação contralateral. Resultados: O efeito inibitório da via eferente nas otoemissões foi diferente (p = 0,012) entre os grupos, sendo observada uma redução média de 1,48 dB SPL nos nascimentos a termo e 1,02 dBSPL no grupo pretermo para o estímulo clique não-linear. Conclusão: Os resultados sugerem efeito inibitório do sistema eferente olivococlear reduzido sobre as emissões otoacústicas em recém-nascidos prematuros.

Introduction: The otoacoustic emissions test is an essential tool in the evaluation of auditory function, since it allows the early detection of cochlear damage of occupational origin. Objective: To present a review of the literature and analyze the effectiveness of the clinical application of the otoacoustic emissions test in workers exposed to noise. Methods: A bibliographical search covering a period of 10 years was performed in the Virtual Health Library including published articles in national and international journals indexed in the internationally recognized databases for the health sciences, LILACS, SCIELO, and MEDLINE, using the terms "otoacoustic emissions" and "occupational exposure." The type of published article (national/international), the type and intensity of the stimulus most commonly used for the evoked otoacoustic emissions, the gender and age of the subjects, and the conclusions from the retrospective studies were all taken into consideration. Results and Conclusions: A total of 19 articles were analyzed, 7 national and 12 international, covering subjects from 17 to 77 years of age, mostly men. The type of stimulus most commonly used for the evoked otoacoustic emissions was the distortion method (12). Through this review, we have concluded that testing of evoked otoacoustic emissions in workers exposed to noise is an important tool in the early diagnosis of noise-induced cochlear hearing disorders...

Os jovens estão cada vez mais expostos à música alta, que pode prejudicar a audição. O teste das Emissões Otoacústicas, por ser mais sensível à exposição ao ruído, permite detectar precocemente alterações cocleares. OBJETIVO: Investigar a prevalência de lesão das células ciliadas externas por meio do teste de emissões otoacústicas em uma amostra de estudantes. MATERIAL E MÉTODO: Foram realizados os testes de emissões otoacústicas por estímulo transiente e por produto de distorção em 134 indivíduos. Os exames foram analisados de acordo com o critério "passa/falha". Tipo de estudo: Estudo seccional descritivo de prevalência. RESULTADOS: Dos 134 participantes, 80,6% apresentaram emissões otoacústicas transiente alteradas, sendo a maioria do gênero masculino; 97,8% apresentaram emissões otoacústicas produto de distorção alterada e 79,9% apresentaram alteração tanto em transiente quanto em produto de distorção, sendo a maioria do gênero masculino e, ainda, 94,0% relataram fazer uso de fones de ouvido; e 82,8% declararam frequentar lugar com música amplificada. CONCLUSÃO: A alta prevalência de testes alterados pode indicar precocemente uma disfunção coclear e, pelo alto número de participantes que relatou exposição à música alta, há a suspeita de que esse hábito pode estar provocando as alterações cocleares.


Exposure to loud music is increasing among young people, and so could be the number of hearing impairment cases in this population. Otoacoustic emission tests are sensitive in capturing the effects of exposure to noise, and allow the detection of early cochlear disorders. OBJECTIVE: This study aims to look into the prevalence rates of injuries to outer hair cells in a population of students through otoacoustic emission testing. MATERIALS AND METHOD: One-hundred and thirty-four subjects were submitted to transient evoked and distortion product otoacoustic emission tests. Subjects were assessed on a "pass/fail" scale. This is a cross-sectional descriptive study on prevalence rates. RESULTS: More than four fifths (80.6%) of the 134 subjects had altered transient otoacoustic emissions, most of whom were males; 97.8% had altered distortion product otoacoustic emissions and 79.9% had altered test results in both transient evoked and distortion product OAEs; most were males; 94.0% reported they used earphones; and 82.8% stated they frequented places where loud music was played. CONCLUSION: The high prevalence rates of altered test results seem to indicate the presence of early cochlear disorders in the studied subjects. A significant number of subjects reported exposure to loud music, a habit that may be conducive to the onset of cochlear disorders.

ABSTRACT

INTRODUÇÃO: Solventes orgânicos são cada vez mais estudados em função de sua ação ototóxica. OBJETIVO: Avaliar a condição das células ciliadas externas e o sistema olivococlear medial de sujeitos expostos a solventes orgânicos. MÉTODO: Trata-se de um estudo prospectivo. Foram avaliados 78 frentistas de três postos de gasolina da cidade de Santa Maria, Rio Grande do Sul (RS) expostos a solventes orgânicos. Após a aplicação dos critérios de inclusão, a amostra ficou constituída por 24 sujeitos. Os procedimentos utilizados na avaliação foram anamnese audiológica, Emissões Otoacústicas Transientes (EOAT) e pesquisa do efeito supressor das EOAT. Um grupo controle (GC) composto por 23 sujeitos, foi utilizado para comparar sujeitos expostos e não-expostos. A coleta de dados foi realizada na sala de Fonoaudiologia do Centro de Referência em Saúde do Trabalhador de Santa Maria. RESULTADOS: A presença de EOAT foi maior na orelha esquerda em ambos os grupos; a média da relação sinal/ruído das EOAT de ambas as orelhas foi maior no GE; o efeito supressor das EOAT na orelha direita foi maior nos sujeitos do GE (62,5%) e na orelha esquerda foi superior no GC (86,96%), com diferença estatisticamente significante. A mediana da relação sinal/ruído das EOAT, segundo a faixa de frequência, foi maior no GC em três faixas de frequência na orelha direita e em uma na orelha esquerda. CONCLUSÃO: Não foram encontrados sinais de alteração nas células ciliadas externas nem no sistema olivococlear medial nos sujeitos expostos a solventes orgânicos.


INTRODUCTION: Organic solvents have been increasingly studied due to its ototoxic action. OBJECTIVE: Evaluate the conditions of outer hair cells and olivocochlear system in individuals exposed to organic solvents. METHOD: This is a prospective study. 78 gas station attendants exposed to organic solvents had been evaluated from three gas stations from Santa Maria city, Rio Grande do Sul (RS). After applying the inclusion criteria, the sample was constituted by 24 individuals. The procedures used on the evaluation were audiological anamnesis, Transient otoacoustic emissions (TEOAES) and research for the suppressive effect of TEOAES. A group control (GC) compounded by 23 individuals was compared to individuals exposed and non-exposed individuals. The data collection has been done in the room of Speech Therapy of Workers Health Reference Center of Santa Maria. RESULTS: The TEOAES presence was major in the left ear in both groups; the average relation of TEOAES signal/noise in both ears was greater in GE; the TEOAES suppressive effect in the right ear was higher in the individual of GE (62,5%) and in the left ear was superior in GC (86,96%), with statistically significant difference. The median sign/noise ratio of TEOAES, according to the frequency range, it was higher in GC in three frequencies ranges in the right ear and one in the left ear. CONCLUSION: It was not found signs of alteration on the outer hair cells neither on the olivocochlear medial system in the individuals exposed to organic solvents.

Os objetivos deste estudo foram descrever e analisar as medidas timpanométricas obtidas com sonda de 226 e 1000 Hz; verificar correlação da timpanometria com o resultado das emissões otoacústicas e otoscopia; descrever as alterações. MATERIAL E MÉTODO: Estudo prospectivo, duplo cego, realizado com 70 bebês, entre 7 dias e 1 mês e 13 dias de vida. Foram realizadas as emissões otoacústicas, otoscopia e timpanometria com sonda de 226 Hz e 1000 Hz. RESULTADOS: Observou-se diferença estatisticamente significante (p<0,05) na correlação entre as medidas timpanométricas analisadas com os dois tons de sonda e na correlação entre o resultado das emissões otoacústicas e a compliância obtida. Foi observada normalidade em todos os exames na maioria dos bebês (94,28 por cento); apenas três (4,28 por cento) foram encaminhados ao diagnóstico audiológico e um bebê (1,42 por cento) apresentou alteração de orelha média confirmada pela otoscopia e timpanometria em 1000 Hz. CONCLUSÕES: A timpanometria realizada com tom sonda de 1000 Hz é a mais recomendada para avaliar bebês até 6 meses de idade. São necessários mais estudos com enfoque nas propriedades mecanoacústicas da orelha média de bebês para que a interpretação de exames com este tom de sonda tenha maior confiabilidade.


This study aimed at describing and analyzing tympanometric results obtained with 226Hz and 1000Hz probe tones; checking for correlations between tympanometry, otoacoustic emissions and otoscopic examination; describing abnormal results found in the evaluation procedures. METHODS: Double-blind and prospective study. Our sample included 70 babies, between 7 days and one month and 13 days of age, without risk indicators for hearing loss, evaluated in the State Neonatal Hearing Screening Program. Transient evoked otoacoustic emissions, otoscopic examination and tympanometry with 226Hz and 1000Hz probe tones were used as assessment tools. The study was approved by the Ethics Committee from the institution. RESULTS: Statistically significant differences were observed (p<0.05) in the tympanometric measures correlation and also between transient evoked otoacoustic emissions and compliance obtained with both probe tones. Most test results were within the normal range (94.28 percent). Three children (4.28 percent) were referred to diagnostic follow-up and one (1.42 percent) had middle ear dysfunction confirmed by otoscopy and 1000Hz tympanometry. CONCLUSIONS: 1000Hz tympanometry is the most reliable probe tone used to evaluate children under three months of age. More studies focusing on middle ear acoustics and mechanics are necessary to provide reliable and precise interpretation in the evaluation of middle ear functions in babies.

OBJECTIVE@#To analyze the difference of subjective hearing threshold and objective hearing threshold, and to discuss the importance of standard for hearing evaluation in forensic medicine.@*METHODS@#Three hundred and fifty-five cases (387 ears) of forensic medical identification with hearing impairment were retrospectively analyzed including the items entrusted and hearing test results. All cases were collected from 2004 to 2012 in the forensic science center.@*RESULTS@#In the 387 ears, 218 ears (56.3%) were evaluated the degree of disability and 106 ears (27.4%) were identified the degree of damage. In the disability degree evaluation, the subjective hearing threshold and the objective hearing threshold were significant different in 120 ears (55.0%), while in damage degree evaluation, the subjective hearing threshold and the objective hearing threshold were significant different in 69 ears (65.1%).@*CONCLUSION@#Because of camouflaging or exaggerating the hearing impairment by the wounded, the subjective hearing threshold can't accurately assess the existence and the degree of hearing impairment. In the forensic identification, auditory brainstem response, 40 Hz auditory event related potential and auditory steady-state response should be combined in the application to evaluate the hearing impairment for the wounded in order to ensure the reliability of the evaluation of hearing impairment.

ABSTRACT

PURPOSE: To investigate the social-cognitive performance of children with diagnostics of autism spectrum disorder (ASD), mental disability (MD) and hearing impairment (HI) in two different communicative situations. METHODS: Participated in this study 30 children ages between 3 and 12 years starting speech language therapy processes, divided in three groups: Group 1 - ten children with diagnoses included in the Autism Spectrum Disorders (ASD); Group 2: ten children diagnosed with mental disabilities (MD) and Group 3: ten children diagnosed with hearing impairment (HI). The subjects were assessed in two different communicative situations (group and individual therapy situation) for 12 months. RESULTS: Data regarding the performance of the three groups in the two situations show that G3 had better absolute performance when compared with other groups, with the variable symbolic play as an important differential criterion for the three groups. CONCLUSION: We conclude that the social-cognitive performance can be used as an auxiliary tool for intervention, helping the identification of variables that could interfere in the communicative performance.


OBJETIVO: Verificar o desempenho sócio-cognitivo de crianças com diagnósticos inseridos nos distúrbios do espectro do autismo (DEA), deficiência mental (DM) e deficiência auditiva (DA) em duas diferentes situações comunicativas. MÉTODOS: Participaram desta pesquisa 30 crianças, entre 3 e 12 anos de idade, em início de atendimento fonoaudiológico, que foram divididas em três grupos: Grupo 1: dez crianças com diagnóstico inserido nos distúrbios do espectro do autismo (DEA); Grupo 2: dez crianças com diagnóstico de deficiência mental (DM); Grupo 3: dez crianças com diagnóstico dentro do grupo de deficiência auditiva (DA). Os sujeitos foram analisados em duas situações comunicativas distintas (situação em grupo e situação individual de terapia), durante 12 meses. RESULTADOS: O grupo composto por crianças e adolescentes com diagnóstico de deficiência auditiva apresentou melhor desempenho absoluto quando comparado com os outros grupos, sendo a variável jogo simbólico um importante critério diferencial nos três grupos. CONCLUSÃO: O desempenho sócio-cognitivo pode ser utilizado como instrumento auxiliar no planejamento terapêutico, facilitando a identificação de variáveis que possam interferir no desempenho comunicativo.

Introdução: Mucopolissacaridoses são caracterizadas pela deficiência de enzimas lisossomais que degradam glicosaminoglicanos. O acúmulo destes metabólitos causa alterações variadas, entre elas, alterações otológicas. Objetivo: Avaliar uma série de pacientes com mucopolissacaridose no Hospital Pequeno Príncipe, enfatizando suas alterações audiológicas e estabelecer um protocolo permanente de avaliação desses pacientes. Método: Estudo retrospectivo da avaliação audiológica através de 18 prontuários de pacientes com diagnóstico de mucopolissacaridose atendidos no Hospital Pequeno Príncipe entre janeiro de 2005 e novembro de 2010. Resultados: Foram selecionados sete prontuários, destes, apenas um não apresentava perda auditiva, sendo que a maioria apresentava perda do tipo condutiva ou mista e apenas um perda exclusivamente neurossensorial. Seis destes pacientes também haviam sido submetidos a pelo menos um procedimento otorrinolaringológico. Conclusão: Os pacientes com mucopolissacaridose requerem cuidados permanentes e acompanhamento multidisciplinar. O otorrinolaringologista pediátrico ocupa importante papel tanto na avaliação otológica e audiológica quanto no manejo das vias aéreas, já que quase a totalidade dos pacientes avaliados apresentavam algum tipo de perda auditiva e indicação de procedimentos otorrinolaringológicos. O padrão definido em nosso serviço foi de ao menos uma consulta inicial quando houver suspeita e/ou diagnóstico da MPS e consultas sequenciais, no mínimo semestrais, ou mais frequentes conforme necessidade.


Introduction: Mucopolysaccharidosis are characterized by the deficit of lysosomal enzymes corrupting the glycosaminoglycans. Accumulating these metabolites causes several alterations, such as otologic alterations. Objective: To evaluate a number of patients with mucopolysaccharidosis at Pequeno Principe Hospital, emphasizing their audiologic alterations and establish a permanent evaluation protocol for these patients. Method: A backward-looking study of audiologic evaluation by using records of 18 patients diagnosed with mucopolysaccharidosis and attended at Pequeno Principe Hospital between January 2005 and November 2010. Results: 7 records were selected, out of which only one did not have a hearing loss, and most of them presented conductive or mixed hearing loss, and only one had an exclusively sensorineural loss. Six of these patients had also been submitted to at least one otorhinolaryngological procedure. Conclusion: Mucopolysaccharidosis patients require a permanent care and a multidisciplinary follow-up. The pediatric otorhinolaryngologist plays an important role when performing the otologic and audiologic evaluation and managing the airways, since almost all the patients evaluated presented some kind of hearing loss and an indication of otorhinolaryngologic procedure. The pattern determined in our service was at least an initial consultation when there is a suspicion and/or diagnosis of MPS and sequential consultations that were at least twice a year or more frequent, if necessary.

Pessoas com dificuldade para compreensão de fala com som competitivo podem apresentar ausência do efeito de supressão das emissões otoacústicas e do reflexo acústico. OBJETIVO: Estudar o desempenho do sistema auditivo eferente em indivíduos normo-ouvintes com queixa de reconhecer a fala na presença de ruído. MATERIAL E MÉTODO: Estudo prospectivo, com amostra composta por 50 voluntários, com idades entre 19 e 32 anos, normo-ouvintes, que referiram (com queixa-CQ) ou não (sem queixa-SQ) dificuldade de compreender a fala no ruído. As emissões otoacústicas evocadas produto de distorção (EOAEPDs) foram testadas nas frequências de 1500 a 6000Hz. O reflexo acústico contralateral (RAC) foi pesquisado nas frequências de 500 a 4000Hz. RESULTADOS: Os grupos diferiram estatisticamente quanto à ocorrência de RAC nas orelhas do lado esquerdo na frequência de 4000Hz. Na frequência de 1500 Hz nas orelhas do lado direito constatou-se predomínio estatisticamente significante da ausência do efeito de supressão das EOAEPDs no grupo CQ. Nas orelhas do lado esquerdo, a ausência do efeito de supressão das EOAEPDs foi maior no grupo CQ em 1500Hz e 2000Hz. CONCLUSÃO: Em normo-ouvintes com autorrelato de dificuldade de reconhecer a fala na presença de ruído verificou-se: ausência de supressão das EOAEPDs nas frequências médias em ambas as orelhas e ausência de reflexo acústico contralateral em 4000Hz na orelha esquerda.


Subjects presenting difficulties in understanding speech with competing sounds may have absence of otoacoustic emission suppression and the acoustic reflex. AIM: To study the performance of the efferent auditory system in normal hearing subjects complaining of difficulties to understand speech in noise. MATERIAL AND METHODS: A prospective study comprising 50 normal-hearing subjects aged from 19 to 32 years, reporting difficulties with speech recognition in noise (with complaints - WC) or not (with no complaints - WNC). Distortion product evoked otoacoustic emissions (DPOAEs) were tested at frequencies from 1500 to 6000 Hz.The contralateral acoustic reflex (CAR) was investigated from 500 to 4000 Hz. RESULTS: Groups differed statistically as to the occurrence of CAR in the left ear at 4000 Hz. At 1500 Hz, there was a statistically significant effect - absence of DPOAEs in the WC group in the right ear. In left ears, absence of DPOAE suppression was higher in the WC at 1500 Hz and 2000 Hz. CONCLUSION: An association between self-reported difficulties in discriminating speech in noise and the absence of contralateral acoustic reflex at 4000 Hz in the left ear was observed; there was also absence of the suppression effect of DPOAEs, especially at middle frequencies in both ears.

TEMA: a habilidade de discriminação auditiva em crianças com desvio fonológico evolutivo. OBJETIVO: investigar esta habilidade nestas crianças, que recebiam ou haviam recebido tratamento fonoaudiológico; verificar se os fonemas alterados no sistema fonológico eram os mesmos não discriminados no teste de figuras para discriminação auditiva (adaptado por Mota et al. 2000, do "The Boston University Speech Sound Discrimination Picture Test") e se as habilidades de discriminação auditiva relacionavam-se com o sexo, a idade e o grau de severidade do desvio fonológico evolutivo. MÉTODO: os dados utilizados foram referentes a 41 crianças, sendo 16 do sexo feminino e 25 do sexo masculino, com idades compreendidas entre 4 anos e 8 anos e 2 meses, e foram coletados por meio da avaliação fonológica da criança e do teste de figuras para discriminação auditiva. RESULTADOS: observou-se que não houve diferença estatisticamente significante de desempenho no teste de figuras para discriminação auditiva entre os sexos e que o melhor desempenho no teste está correlacionado estatisticamente com o avanço da idade cronológica. Quanto mais severo for o desvio fonológico evolutivo maior é o número de fonemas para os quais a criança apresenta inabilidade de discriminação auditiva. CONCLUSÃO: a inabilidade de discriminação auditiva pode ser um fator causal ou agravante do desvio fonológico evolutivo, embora não se aplique a todos os casos.


BACKGROUND: hearing discrimination abilities in children with phonological disorders. AIM: to investigate the ability of hearing discrimination in children with Phonological Disorders who received or were receiving phonological treatment; to verify if the altered phonemes were the same as those which were not discriminated in the Picture Test for Hearing Discrimination (adapted for Portuguese language by Mota et al 2000, based on "The Boston University Speech Sound Discrimination Picture Test") and to verify if the ability of hearing discrimination is related to gender, age and the phonological disorder severity level. METHOD: 41 children, 16 females and 25 males, with ages ranging between 4 and 8.2 years were evaluated. Data used for analyses were taken from the results obtained in the Picture Test for Hearing Discrimination and from the Phonological Assessment. RESULTS: no statistically significant difference between genders Statistical analyses also indicated that the performance in the Picture Test for Hearing Discrimination was correlated to age. The more severe the phonological disorder the higher the number of phonemes that were not discriminated. CONCLUSIONS: hearing discrimination inability can be a causative or aggravating factor of phonological disorder, although this rule does not apply to all cases.

ABSTRACT

Introdução: As disfunções da articulação temporomandibular (ATM) abrangem uma série de problemas clínicos que envolvem a musculatura mastigatória, a própria articulação e estruturas associadas à região da orelha. Objetivo: Analisar a relação entre disfunção temporomandibular e sintomas otológicos. Método: Fizeram parte do estudo retrospectivo 20 mulheres com média de idade de 22,38 anos. O estudo foi realizado por uma equipe multidisciplinar que envolveu médico otorrinolaringologista, fonoaudiólogas e ortodontista. As participantes foram submetidas ao preenchimento de um questionário que abordou questões sobre sintomas otológicos e posteriormente foram submetidas a um exame otoscópico e exame clínico da articulação temporomandibular. Resultados: 85% da amostra tiveram disfunção temporomandibular conjuntamente com queixas de sintomas otológicos. Os sintomas otológicos mais autorreferidos foram de plenitude auricular (50%), zumbido (35%) e vertigem (10%). Conclusão: Os resultados mostraram alto percentual entre as DTMs e sintomas otológicos. Outras pesquisas com uma amostra maior são sugeridas com o intuito de confirmar a correlação entre DTM e Sintomas otológicos.


Introduction: The temporomandibular joint dysfunctions (TMJ) include several clinical problems which wrap the masticatory muscle, the joint itself and structures associated to the ear region. Objective: To analyze the relation between temporomandibular dysfunction and otological symptoms. Method: Were part of a retrospective study 20 women with mean age of 22, 38 years. This study was realized by a multidisciplinary team that wrapped otorhinolaryngologist, speech therapist and orthodontist. The participants were submitted to the filling of a questionnaire that addressed questions about otological symptoms and later they were submitted to an otoscopy exam and a clinical examination of the temporomandibular joint. Results: Eighty-five per cent had simultaneously temporomandibular dysfunction with otological symptoms complaints. The otological symptoms more mentioned were auricular plenitude (50%), tinnitus (35%), and dizziness (10%). Conclusion: The results showed high percentage between DTMs and otological symptoms. Other researches with a larger sample are suggested with the aim of confirm the correlation between DTM and otological symptoms.

ABSTRACT

OBJECTIVE@#To explore characteristics and causes of equilibrium function deficits after traumatic brain injury(TBI).@*METHODS@#Ninety-five patients after TBI in traffic accidents were tested using computerized dynamic posturography (CDP). The CDP findings of patients were compared with normal value. The patients were grouped based on TBI and audition disorders degrees. The results were compared within the patients groups.@*RESULTS@#The equilibrium scores of the TBI group were significantly lower than the normal value. The utilization rate decreased for vision and vestibular sensation information in the TBI group, especially for vestibular sensation. The primary TBI degree and audition decrease had no significant affect on posture stability.@*CONCLUSION@#The poor posture stability was observed in TBI patients without limbs disturbance. The response and adaptation abilities decrease in these patients. This could be explained by the damage to peripheral vestibular and central balance structures.
